4 holes in the firewall/tunnel. The bottom two bolts I can get to from under the car.

Depending on what your carpet, etc., looks like, you can cut an 'X' over the holes to get a socket through...

Here is mine sitting on the frame rails and tied to the strut bars.

As far as motor plate/mid plate mounting, you need to either use the mid plate with a motor plate OR use the motor plate and stock mounts. The mid plate supports the back of the block. Stock mounts mount almost mid way on the block and take a LOT of the load from the motor/trans joint. Running a motor plate and just stock trans mount, can lead to a cracked bellhousing.
